The first recorded encounter of a Reborn Warrior was in cloud city on Bespin. Kyle Katarn battled with and defeated the would-be assasin in lightsaber combat. This first Reborn was not in actuality the first Reborn engineered by the Empire. In fact, records show that the very first Reborn warrior was still being trained as Kyle was battling the Reborn warrior on Bespin.
